Need based scholarships are available and awarded based on the need of the player’s parent(s) or guardian(s). A written application is required and must be returned to DUSA. All completed applications will be reviewed by the DUSA Scholarship Committee and will be held strictly confidential. Applications for these scholarships are accepted on a continuing rotation but are on a first-come first-served basis when awarding amounts.  Scholarship's are available and awarded from 0-100% coverage of dues, uniforms, and cost. Applicant's awarded scholarships will be required to volunteer a set number of hours (such as concessions) depending on the amount of award. Any member of the family will be able to fill the volunteer hours, all hours will be documented for the benefit of all involved.

Any extenuating family circumstances should be attached to this form in the. Along with this application all documentation must be returned for application to be considered by the scholarship committee. Documentation including any applicants SS# should make sure the SS# is removed or blackened out before submitting for security reasons.
